Student Registration
Documents needed for registration:
Proof of residency:
1. Valid rental or lease agreement and one current home utility bill (gas or electric)
Or
2. Current property tax statement or settlement statement and one current home utility bill (gas or electric)
· Immunization certificate (Ga. Form 3231)
· Ear, eye, dental, and nutrition form (Ga. Form 3300)
· Student’s birth certificate
· Student’s social security card
